Home health is short term, skilled medical care provided in your home when it is hard to leave and you need a nurse or therapist to help with a specific health problem. The goal is to help you recover, improve, or better manage a serious condition at home, so you can avoid unnecessary trips to the hospital and keep as much independence as possible.

Determining If Home Health Is the Right Choice for You

Home health may be right for you if you have a serious illness or recent hospital stay, it is a real effort to leave home, and your provider believes you need parttime visits from a nurse or therapist to meet medical goals. Visits are not 24hour care; they are planned, intermittent visits to check on your condition, adjust your care, and teach you and your family what to do between visits.

How to Begin Your Home Health Care Journey

Home health typically includes:

  • A skilled team: registered nurse, physical therapist, occupational therapist, speech therapist, and sometimes social worker and home health aide.
  • Medical care at home: checking your condition, managing wounds, monitoring heart or lung problems, helping with new medicines, and watching for changes that need attention.
  • Rehabilitation: help with strength, balance, walking, bathing, dressing, and daily activities after illness, injury, or surgery.

In some situations, limited nonmedical help, such as a home health aide or brief homemaking support, may also be available as part of the plan of care. Your nurse or therapist will explain what is included for you.

Talk with us

  • Call our home health office to ask questions and share what has been happening.

Review with your provider

  • With your permission, we review your medical needs with your provider to see whether home health is appropriate.

Decide together

  • If home health seems right, you, your family, and your provider decide together. We review the forms in plain language and outline the visit plan.

Begin visits

  • Once ordered, your home health team begins scheduled visits based on what is needed. You receive a number to call with questions or concerns between visits.

You do not have to figure this out alone; our home health team works closely with your trusted providers to support you at home.
