Home health is short term, skilled medical care provided in your home when it is hard to leave and you need a nurse or therapist to help with a specific health problem. The goal is to help you recover, improve, or better manage a serious condition at home, so you can avoid unnecessary trips to the hospital and keep as much independence as possible.
Home health may be right for you if you have a serious illness or recent hospital stay, it is a real effort to leave home, and your provider believes you need parttime visits from a nurse or therapist to meet medical goals. Visits are not 24hour care; they are planned, intermittent visits to check on your condition, adjust your care, and teach you and your family what to do between visits.
